ADC 2023 Speaker Reimbursement
Thank you for presenting at the Annual Dialysis Conference 2023.
For reimbursement you may download the forms below.
You will have 30 days to complete the forms for reimbursement and return to us at Conferences@mimh.edu
When completing your reimbursement paperwork –
- Please note the meal allowances for lunch and dinner.
- Please sign (all documents) with a digital signature (do not type) or print, sign and rescan.
- Make sure your math is correct.
- Make sure all receipts are attached.
- For wire transfer forms, you may need assistance from your bank to complete.
If forms are not completed correctly, University accounting will return to you for corrections.
