This session will discuss optimal decision-making process for patients with advanced kidney disease in the choice of treatment.  Topics will address the benefits of CKD education, shared decision-making, conservative management, incremental treatments as a bridge to transplantation and dialysis (in-center or home dialysis) and modality transitions.
