Home Hemodialysis prescription is more than just adjusting treatments to achieve target Urea Kt/V.  Home HD prescription should include goals to achieve adequate small and middle solute clearances, optimum volume management and residual renal function.  The prescription must also be patient-centric considering timing of the therapy and logistical concerns that addresses maintaining a quality of life that minimizes major disruptions to the patient’s lifestyle.

Patients initiating peritoneal dialysis (PD) therapy are given the option to choose either Continuous Ambulatory Peritoneal Dialysis (CAPD)or Automated PD (APD). Each option offers specific benefits and medical indications considering the patient’s medical condition. Unless there is a medical contraindication, providers often leave the choice of therapy to patients to suit their lifestyle. For patients who stay on long-term PD, it is strongly recommended that a continuous form of therapy is prescribed for the patient – 24 hours a day, 7 days a week. Over the past several decades, there has been a trend to for patients to choose APD over CAPD which does not provide continuous dialysis. Consequently, several undesired problems are being noticed in PD patients on intermittent treatments.,

This workshop will explain various issues highlighted above in a case-based approach. At the end of this workshop, the participant should be able to write a rational, patient-specific prescription for long-term PD.

Objective:  The dialysis access is a cornerstone for the patients to continue both Home and Incenter Dialysis. A deeper understanding of AV access and PD catheter is essential for physicians and the interdisciplinary team to care for patients and triage in the event of complications. This course will focus on physical examination and point of care evaluation of both PD catheters and AV access along with techniques to train Home HD patients and care partner on cannulation. The participants will have the opportunity to practice on both human and synthetic models using high-quality ultrasound machines supervised by experienced faculty.  Training sessions give you practical, hands-on experience.

Total duration 3 hours and 15 minutes.  The course will start with a brief didactic presentation followed by a hands-on learning experience.  This course will focus on:

This session will briefly review the concept, purpose and procedure and interpretation of the standard procedure of the peritoneal equilibration test.  The primary focus of the session will illustrate how PET results can be obtained without expending the traditional nursing time to do the standard procedure.
